I hereby certify that all of the information provided in this cancellation request is true to the best of my knowledge. I also understand that this registration may not be re-instated at any time and that no refunds may be received from this registration.

NOTE: Only persons named as owner/co-owner can request a plate to be cancelled, or submit Power of Attorney documents. In the case of Owner Death: A copy of death certificate, and Probate Court documentation will need to be submitted.
